Florida financial affidavit, section by section

Published October 27, 2026

This guide walks through Florida's Family Law Financial Affidavit in order, covering both versions: the short form, Form 12.902(b) (revision 10/21), and the long form, Form 12.902(c) (revision 06/25). Line numbers and labels are the forms' own. It is general information, not legal advice; for which form applies to you, see short form versus long form.

Section I: income, lines 1 to 17

Both forms open the income section with: "All amounts must be MONTHLY." and "Items included under “other” should be listed separately with separate dollar amounts." The lines are almost identical (the short form puts "Monthly" in front of each label):

LineIncome
1Gross salary or wages
2Bonuses, commissions, allowances, overtime, tips, and similar payments
3Business income (gross receipts minus ordinary and necessary expenses required to produce income), with an itemized sheet
4 to 8Disability benefits/SSI; workers' compensation; unemployment compensation (short form) or reemployment assistance (long form); pension, retirement, or annuity payments; Social Security benefits
9Alimony actually received: 9a from this case, 9b from other cases
10 to 13Interest and dividends; rental income (net, with a sheet); royalties, trusts, or estates; reimbursed expenses and in-kind payments to the extent they reduce personal living expenses
14Gains derived from dealing in property (not including nonrecurring gains)
15, 16Any other income of a recurring nature, with its source
17TOTAL PRESENT MONTHLY GROSS INCOME (lines 1 to 16)

Before line 1, the short form asks for occupation, employer and pay rate. The long form also asks your age, whether you are unemployed, employed or retired, whether you expect to change jobs, and last year's gross income for you and, if known, the other party.

Deductions, lines 18 to 27

Both forms: 18 income tax (with filing status and dependents), 19 FICA or self-employment taxes, 20 Medicare, 21 mandatory union dues, 22 mandatory retirement payments, 23 health insurance (the long form says "for you only"), 24 court-ordered child support paid for children from another relationship, 25 court-ordered alimony paid. Line 26 totals the deductions and line 27 is "PRESENT NET MONTHLY INCOME" (line 17 minus line 26).

Have your statements as PDF, CSV or OFX? Financial Affidavit Numbers reads them in your browser, checks that each statement reconciles, and sorts the transactions into the form's own lines. You confirm every category and decide every flagged item. The worksheet gives each line's monthly average and the statement lines behind it:
WORKSHEET: FORM 12.902(c), SECTION II, PRIMARY HOUSEHOLD
14 Food and home supplies $612.40
average monthly, 12 months, 148 transactions, schedule p. 4
15 Meals outside home $187.15
average monthly, 12 months, 61 transactions, schedule p. 5
5 Electricity $142.60
average monthly, 12 months, 12 transactions, schedule p. 5

Section II: average monthly expenses

Both forms say that if your expenses "do not reflect what you actually pay currently, you should write “estimate” next to each amount that is estimated." (The short form limits that to dissolution of marriage cases.)

Short form. Six groups with named lines and blanks: A. Household (mortgage or rent, property taxes, utilities, telephone, food, meals outside home, maintenance/repairs); B. Automobile; C. Child(ren)'s expenses; D. Insurance; E. Other expenses not listed above; F. Payments to creditors. Line 28 is the total.

Long form. Numbered lines in seven groups, each with a subtotal: primary household 1 to 26 (subtotal 27); automobiles 28 to 38 (39); children common to both parties 40 to 64 (65); children from another relationship 66 to 69 (70); insurance 71 to 78 (79); other expenses 80 to 104 (105); payments to creditors 106 to 118 (119). Line 120 adds the subtotals. As printed, line 39 reads "add lines 28 through 35" although the automobile lines run to 38.

The 06/25 long form renumbered this section. Compared with the 11/20 long form, whose total was line 105, it adds lines such as subscription streaming services (17), communication technology/remote working subscriptions (21), electric car charging outside the home (29), automobile subscriptions and roadside services (37), remote learning (62), pet care (87), gambling and lottery (93), tobacco, alcohol, and vaping (94), and attorney fees and court costs (95). If you are working from an older copy, the line numbers will not match.

Payments to creditors on the long form carry an instruction: "List only when payments are currently made by you on outstanding balances and not listed elsewhere on this affidavit." See credit card payments versus purchases for why that matters.

Summary

Net income against expenses: lines 29 to 32 on the short form, 121 to 124 on the long form. One line is the surplus if net income is larger, the other is the deficit if expenses are larger.

Section III: assets and liabilities

The short form lists assets (cash, bank accounts, stocks, real estate, cars, retirement plans) and liabilities (mortgages, auto loans, charge and credit card accounts) with a current value and a nonmarital column. The long form adds four steps of instructions, separate marital and nonmarital columns, and new asset lines including "Cash (in digital wallets, including but not limited to Venmo, Apple Wallet, and PayPal)", virtual currency and NFTs, followed by a net worth section. It also says: "If you have an equitable distribution schedule or balance sheet that includes the information below, you may attach the document instead of filling out Section III." Both forms then list contingent assets and liabilities, such as a possible bonus or inheritance, or future unpaid taxes. Whether an item is marital or nonmarital is a legal question; the forms point to section 61.075(1), Florida Statutes.

The last page

A checkbox says whether a Child Support Guidelines Worksheet (Form 12.902(e)) is or will be filed, a certificate of service, and your signature under penalty of perjury. If a nonlawyer helped you, the bottom block is for their details.

Financial Affidavit Numbers covers Sections I and II: from your statements it gives a monthly figure for each income and expense line of the form you choose, with the transactions behind it. You decide every category and copy the figures onto the form; it does not fill in the affidavit or touch Section III.

Common questions

What are the sections of the Florida financial affidavit?

Section I is income (lines 1 to 17) and deductions (18 to 27, ending with net monthly income). Section II is average monthly expenses, followed by a summary of surplus or deficit. Section III is assets and liabilities, including contingent ones. The last page covers the child support guidelines worksheet, service and your signature.

What changed in the 06/25 Florida financial affidavit long form?

Compared with the 11/20 long form, Section II was renumbered (the total moved from line 105 to line 120) and lines were added, such as subscription streaming services, electric car charging, gambling and lottery, and tobacco, alcohol, and vaping. Assets now include digital wallets and virtual currency.

Where does net income go on the Florida financial affidavit?

Line 27, 'PRESENT NET MONTHLY INCOME', is line 17 (gross income) minus line 26 (total deductions). The summary carries it forward to line 29 on the short form or line 121 on the long form.

Do you have to fill out Section III of the long form if you have a balance sheet?

The long form says that if you have an equitable distribution schedule or balance sheet that includes the information, you may attach it instead of filling out Section III.
