Colorado Sworn Financial Statement (JDF 1111), line by line

Published October 28, 2026

Colorado's Sworn Financial Statement is form JDF 1111SC, which the Colorado Rules of Civil Procedure also print as Form 35.2. The current version is marked R10/25. A companion, JDF 1111SS (Supporting Schedules), is attached when you own the kinds of assets the form sends there. This guide walks through the form in order, with its own wording. It is general information, not legal advice; ask a lawyer or your court's self-help center about your case.

The first page: work and household

Before any numbers, the form asks whether you are employed, hours per week, how often you are paid (weekly, bi-weekly, twice a month or monthly), whether pay is a monthly salary or an hourly rate, your occupation and employer, and, if unemployed, when you last worked and why. Then: how many adults and minor children are in the household, what you believe the other party's monthly gross income is, and each party's annual gross income for the last tax year.

1. Monthly income

The heading says: "Convert annual, bi-monthly, and weekly amounts to monthly amounts." The lines:

GroupLines on the form

The first line gathers pay of every kind, before taxes, with self-employment and business income: one figure rather than separate lines for overtime or bonuses. Total Income adds Total Monthly Income and Total Monthly Miscellaneous Income.

Have your statements as PDF, CSV or OFX? Financial Affidavit Numbers reads them in your browser, checks that each statement reconciles, and sorts the transactions into the form's own lines. You confirm every category and decide every flagged item. The worksheet gives each line's monthly average and the statement lines behind it:

2. Monthly deductions

Mandatory deductions are federal income tax, state/local income tax, PERA/Civil Service, Social Security tax, Medicare tax and other. Voluntary deductions are life and disability insurance, health, dental and vision insurance premiums (with the number of people covered), child care deducted from salary, a flex benefit cafeteria plan, stocks and bonds, and retirement and deferred compensation. These come from pay stubs, and they are why section 3 says to list only expenses "not identified in the deductions above".

3. Monthly expenses, A to I

The note at the top: "List regular monthly expenses below that you pay on an on-going basis and that are not identified in the deductions above."

4. Debts, the summary and assets

Section 4 lists unsecured debts (credit cards, store accounts, loans from family, back taxes) with the minimum monthly payment required, and says not to list liens such as mortgages and car loans, which already appear above. The summary then takes Total Income, subtracts deductions to get net income, subtracts expenses and minimum debt payments, and shows a Net Excess or Shortfall. Section 5 lists assets by petitioner, co-petitioner/respondent or joint. The statement ends: "I declare under penalty of perjury under the law of Colorado that the foregoing is true and correct." For when it is due, see the 42-day deadline and monthly figures.

Financial Affidavit Numbers works out monthly figures for sections 1, 3 and 4 from your bank and card statements, line by line, with the transactions behind each one. You decide every category and copy the figures onto JDF 1111 yourself; it does not fill in the statement.
